Duties
- The incumbent practices in the Medicine Hematology Oncology Service as a Nurse Practitioner (NP) to provide a full range of services emphasizing quality - timeliness - and the advancement of the Veterans Affairs (VA) mission
- The NP provides advanced clinical practice - consultation - and management: In collaboration with attending physicians Expected to evaluate - diagnose and develop treatment/management plans for patients with hematological and oncological conditions including but not limited to hematological malignancies - oncological malignancies - and benign hematology
- - It would entail performing comprehensive disease specific history and physical exam in outpatient clinical setting as well as 9E infusion - ordering and interpreting diagnostic exams - labs - and imaging - and establishment of medical diagnosis with appropriate management of such diagnosis
- The NP practices to the full extent of their education - training - and certification
- Core functions are outlined in the clinical privileges that correspond with this position to render the following services to oncological patients as appropriate per scope of practice: Patient care - Oncology workflow care - 9E infusion care - Education and Emergencies PATIENT CARE 1
- NP is responsible for
- development and/or ordering of therapeutic plan of care including prescriptions (scheduled narcotics - oral chemotherapy drugs - anticoagulants etc.) as directed by scope of practice

- NP is required to
- counseling and education of patient/family member regarding plan of care including disease process - treatment plans - and medication regimens
- Review medications for clinical need - simplification and possible drug-drug interactions and adjust as indicated

- NP is expected for
- recognition - patient education - management - and treatment of chemotherapy (oral and IV) side effects - infusion reactions - and toxicities

- NP is expected to
- perform invasive procedures where applicable and as per scope of practice (chemotherapy extravasation - port care)
- ONCOLOGY WORKFLOW 1
- NP will collaborate with supervising attendings as well as fellows regarding admission - discharge and transfer decisions for oncological patient to facilitate patient flow - including inpatient - outpatient and chemotherapy infusion center

- NP is expected to facilitate patient messages via phone as well as message portal
- 3
- NP must be able to collaborate with the multidisciplinary team including case management - primary care and specialty services to expediate oncology patient care
- 4
- NP may need close collaboration with palliative care and social services to provide holistic needs of cancer patients including hospice discussions when appropriate
- 9E INFUSION 1
- With appropriate supervision - NP should be able to provide ongoing support/assessment of patients receiving chemotherapy and other infusions including toxicity - infusion reactions - side effect management - acute clinical needs or other complaints as appropriate
- This may entail ordering supportive therapies (IVF - anti-emetics - electrolytes - etc)

- Assess and manage transfusion reactions

- Triage and assessment of chemotherapy cancellations - patient calls for illness or uncontrolled symptoms

- Assist with acute direct admissions for toxicity management
- 5
- Discuss and coordinate \"add-on\" patients as requested by Hem/Onc attendings/Fellows as needed
- EDUCATION/RESEARCH 1
- Provide formal and informal educational programs for other members of the health care team consistent with evidence-based practice standards

- Participate in mentoring activities including preceptorship of nursing students
- 3
- Functions as a clinical resource for other team members including but not limited to interns - residents - fellows - nurses - and medical students
- 4
- Works with physician leaders and hospital administration to achieve service specific outcome targets for key metrics including clinical outcomes - LOS - cost of care and patient satisfaction

- Actively participates and contributes to various department and division meetings and organizational initiatives
- Other Education/Research duties - as well as other duties as assigned
